What Is IUI?

IUI stands for Intrauterine Insemination. It is a type of fertility treatment in which sperm is directly inserted into a woman’s uterus to increase the chances of fertilization. IUI can be used in conjunction with fertility medications to stimulate ovulation, and is often recommended for couples experiencing male infertility, unexplained infertility, or difficulty conceiving due to cervical issues.

10 Steps to Successful IUI

  1. Tablets which increase the number of eggs are prescribed at the start of the menstrual cycle and injections if required are given on daily basis for 4-5days to stimulate the ovaries to produce follicles and cause ovulation.
  2. A week after drug treatment has commenced, you will be invited to have regular ultrasound scans to check the development and growth of any follicles.
  3. When your ultrasound scan shows one to three follicles that have reached their target size, you will be given  trigger injection to cause release of egg. Your IUI procedure will be timed to coincide with this.
  4. Your partner will be asked to provide a sperm sample on the morning of the IUI procedure. The sample is washed and prepared in the laboratory for use later that day.
  5. During the IUI procedure a fine catheter will be inserted through the cervix into the uterus where the prepared sperm will be passed. You will be asked to rest for at least 30 minutes following this, before you go home.
  6. It is normal to get some abdominal cramps or discomfort during or just after the procedure, these should quickly wear off.You may resume normal activities after 24 hours.
  7. You will be asked to carry out a pregnancy test two weeks following the IUI unless your period has already started.
  8. If the test is positive, we will arrange a scan for you in a couple of weeks to ensure all is well.

How Is IUI Different From IVF?

IUI (intrauterine insemination) and IVF (in vitro fertilization) are two different fertility treatments. IUI involves the placement of sperm directly into the uterus, while IVF involves the fertilization of an egg with sperm outside the body in a laboratory and then the transfer of the resulting embryo(s) into the uterus.

In IUI, sperm is collected and washed in the laboratory to remove any impurities, and then inserted directly into the uterus using a thin, flexible catheter. This increases the chances of fertilization by bringing the sperm closer to the egg. IUI is typically used for couples with male factor infertility, unexplained infertility, or for single women or same-sex female couples who wish to conceive with donor sperm.

In contrast, IVF involves retrieving eggs from the woman’s ovaries, fertilizing them with sperm in a laboratory dish, and then transferring the resulting embryo(s) into the uterus. IVF is typically used for couples with more complex fertility issues, such as blocked fallopian tubes or severe male factor infertility. IVF also allows for genetic testing of embryos before transfer, which can help reduce the risk of certain genetic disorders.

FOLLICULAR MONITORING

To maximise the chance of conceiving it is crucial that intercourse is timed to coincide with ovulation following treatment. We will therefore monitor your response to treatment, especially with gonadotroph in injections be it for timed intercourse or IUI or IVF.

TIMED INTERCOURSE: Sexual intercourse can therefore be timed to coincide with the release of the egg. In general, sexual intercourse about 3 times a week during this period will ensure that there is a good supply of sperm into the genital tract to fertilise the egg when it is released. Long periods of abstinence from intercourse are not encouraged as this can affect sperm function.

Monitoring Egg Development

In a normal menstrual cycle, several follicles (little sacs of fluid each containing one egg) begin to grow in each ovary. Usually only one grows big enough to ovulate. The other follicles normally die. With induction of ovulation,we try to make more number of eggs.

1. What is the monitoring for?

The aim of treatment is to increase quantity of eggs so as to increase chances of fertilisation where as simultaneously decreasing the associated risk such as OHSS (development of too many eggs ) and  multiple pregnancies

2. How is monitoring done?

This is performed by ultrasound scans which are safe, simple and fast. Scanning of the ovaries is performed through the abdomen or through the vagina. An instrument called an ultrasound probe is placed on your abdomen (a full bladder is necessary). The probe then transmits a picture on to a small television screen so the ovarian follicles can be seen and measured. For vaginal scans a special probe will be placed in the vagina to obtain pictures of the ovaries. The vaginal probe is less than the thickness of two fingers and should not hurt you, (a full bladder is not required).

3. What if more than one egg develop?

If a follicle grows to about 18-26mm in size, it is likely to produce an egg. If it is less than 14mm, it is not likely to produce an egg. When the likelihood of ovarian hyperstimulation syndrome is detected on ultrasound scan, treatment will be stopped as it can be life threatening in severe cases and we will not proceed to administer HCG injection .You will also be requested to avoid intercourse or use effective method of contraception as pregnancy can aggravate the condition.

4. What other problems could stop treatment in a cycle?

Sometimes the ovaries respond inadequately to the drugs and this will result in the treatment cycle being cancelled. A further attempt would then be arranged and the dose of drug used to stimulate the ovaries increased or another treatment plan organised using different drugs.

Egg Release

Once scans have confirmed that one mature follicle of about 18mm has developed in the ovaries, you will be given another injection called trigger injection which is Human Chorionic Gonadotrophin (HCG) to cause the follicle or egg sac to rupture and release the egg. Following the administration of HCG, it takes between 24 and 48 hours for the egg to be released. Some women know when it has happened because they experience lower abdominal cramp or discomfort.

If I get pregnant, will my baby be healthy?

After induction of ovulation the chance of a miscarriage or a baby with an abnormality is the same as after natural conception. We do however, recommend that if you are planning to become pregnant, you should start folic acid until after 12 weeks of pregnancy. This will help to reduce the risk of some birth defects.
